Here is the fixed crack for Solidworks 2011 provided by SSQ. This was only tested on SP0 x64 version.
This will fix those errors you sometimes get with the x64 version (Error: Can not initialize library (%d, %d, %d)" or " Error: Can not open handle (%d, %d, %d)") etc.
Basic instructions:
1. Disconnect internet connection
2. Install Solidworks 2011 with a key provided in the SSQ NFO.
3. Drag the "setup" folder from "SW2011_SSQ_fixed" to "C:\Program Files\SolidWorks\" and click Yes to merge and then overwrite/replace all files windows asks about
4. Navigate to "C:\Program Files\SolidWorks\setup\i386\x64_fix" and run "ServiceInst.exe"
5. Navigate to "C:\Program Files\SolidWorks\setup\i386" and run "swlmwiz.exe" click yes or ok both times and let the patcher run.
6. Connect internet
Solidworks 2011 should now be patched. My instructions are for x64 version, I have not tested the x86 version.
I think that you will have to run this patch every 30 days to reset the trial timer.
